Cryoluxe™ “Mother's Day, Every Day” Nomination Contest

1. Sponsor

The Cryoluxe™ “Mother's Day, Every Day” Nomination Contest (the “Contest”) is sponsored by Skin Revival Science LLC, a Wyoming limited liability company, located at 30 N Gould Street, Ste R, Sheridan, WY 82801 (“Sponsor”).

2. Eligibility

Nominators. The Contest is open to legal residents of the fifty United States and District of Columbia, excluding residents of New York and Florida, who are at least 18 years old, or the age of majority in their state of residence if higher, at the time the nomination is submitted.

Nominees and Honorees. A person nominated in the Contest (a “Nominee”) must also satisfy the residency and age requirements above in order to be selected as an Honoree. Self-nominations are permitted.

Residents of New York and Florida are not eligible to participate as nominators or Honorees at this time. Nominations for individuals residing in New York or Florida will not be considered.

Sponsor may expand eligibility to additional jurisdictions during the Contest Period by publishing updated Official Rules. Any change in eligibility will apply only to nominations submitted after the updated rules are published.

Employees, officers, members, and contractors of Sponsor and of its agencies and partners, plus their immediate family and household members, cannot be Honorees.

Each person may be selected as an Honoree only once during the Contest Period.

3. Contest Period

Nominations will be accepted beginning on October 7, 2026. The first daily Honoree will be selected for November 1, 2026. The Contest ends at 11:59 p.m. Eastern Time on October 31, 2027, unless extended by Sponsor through publication of updated Official Rules.

Sponsor's computer system will serve as the official timekeeping device for the Contest.

Each calendar day from November 1, 2026 through October 31, 2027 will constitute a “Daily Selection Period.” Sponsor intends to select one Honoree for each Daily Selection Period, for a total of up to 365 Honorees during the Contest Period.

4. How to Nominate a Mom

To submit a nomination, visit: skinrevivalscience.com/pages/mothers-day-every-day

The nomination form may also be accessed through links provided on Skin Revival Science social media accounts.

Each nomination must include:

• The nominator's first and last name and email address;
• The Nominee's first name and state of residence;
• Either: the Nominee's email address, provided the Nominee has agreed that it may be shared with Sponsor; or confirmation that the nominator can put Sponsor directly in contact with the Nominee if she is selected;
• A written nomination of up to 500 words explaining what makes the Nominee special; and
• Optionally, up to 3 photographs of the Nominee or of the Nominee together with the nominator.

There is no limit on the number of different people an individual may nominate. A Nominee may be nominated by more than one person; however, receiving multiple nominations does not increase a Nominee's chances of being selected.

Only nominations submitted through the official nomination form will be considered. Social media comments, direct messages, tags, posts, or other communications are not valid Contest entries.

5. Nomination Requirements

By submitting a nomination, the nominator represents and confirms that:

• The nomination is original and truthful;
• The nominator has permission from every identifiable person shown in any submitted photograph;
• The Nominee has given permission for any contact information provided about her to be shared with Sponsor;
• The nomination does not contain material that is obscene, defamatory, discriminatory, threatening, unlawful, or infringing; and
• The nomination does not advertise or promote another brand, company, product, or service.

Sponsor reserves the right to disqualify any nomination that does not comply with these Official Rules or that is inconsistent with the spirit or purpose of the Contest.

6. How Honorees Are Selected

Honorees are selected based on the quality and content of the nomination and not by chance. For each Daily Selection Period, a panel of judges selected by Sponsor will review eligible nominations received by 11:59 p.m. Eastern Time on the preceding day.

Each eligible nomination will be evaluated using the following criteria:

• Heart of the Story — 50%. How clearly the nomination demonstrates the care, love, generosity, or everyday impact the Nominee has on others.
• Specificity — 30%. The personal and meaningful details that make the Nominee and her story unique.
• Spirit of the Campaign — 20%. How well the nomination reflects the purpose of “Mother's Day, Every Day”: recognizing that moms who care for others deserve to be cared for and celebrated too.

Writing ability, grammar, story length, and photograph quality are not judging criteria. Eligible nominations that are not selected will remain eligible for future Daily Selection Periods through the end of the Contest Period.

The judges' decisions are final. The likelihood of being selected depends on both the number and quality of eligible nominations received.

7. Notification and Verification

Sponsor will contact a selected Nominee, or the nominator if necessary to facilitate contact with the Nominee, by email or through an official Skin Revival Science social media account. A selected Nominee must respond within seven days of the initial notification.

To be confirmed as an Honoree, the selected Nominee must:

• Confirm that she satisfies the eligibility requirements;
• Accept the honor and prize;
• Provide a valid shipping address located in an eligible jurisdiction; and
• Complete any reasonable eligibility or consent documentation requested by Sponsor.

If a selected Nominee cannot be contacted, fails to respond within the required time, is found to be ineligible, or declines the honor or prize, Sponsor may select an alternate Honoree from the remaining eligible nominations.

Sponsor will never require payment, shipping fees, banking information, credit card information, passwords, or other financial information in order to receive a prize. Any communication requesting payment or financial information is not an authorized communication from Sponsor and should be reported to support@skinrevivalscience.com.

8. Prize

Each confirmed Honoree will receive one Eye Revive Care Package consisting of:

• Three Cryoluxe™ Eye Revive in 5 product packs;
• One pack intended for the Honoree; and
• Two additional packs that she may share with people of her choice or enjoy herself.

The Approximate Retail Value (“ARV”) of each Care Package is US$90. Up to 365 Care Packages may be awarded during the Contest Period, for a maximum total ARV of US$32,850.

Prizes will be shipped free of charge to an eligible U.S. address, generally within 14 days after an Honoree has completed verification.

Prizes have no cash alternative, may not be transferred or exchanged for cash, and may be substituted by Sponsor with a prize of equal or greater value if the advertised prize becomes unavailable. Honorees are responsible for any taxes, if applicable. Recipients should review all product packaging, instructions, and directions before use.

9. Privacy, Photos and Publicity

There is no requirement for an Honoree to post on social media, publish a photograph, write or provide a product review, or promote Skin Revival Science or Cryoluxe™.

Sponsor will publicly share an Honoree's name, photograph, nomination story, or other personal information only with the Honoree's express permission. A nominator's name will be shared publicly only with that nominator's permission.

An Honoree may withdraw permission for future public use by contacting support@skinrevivalscience.com, although withdrawal will not affect materials already lawfully published before the request was received.

Personal information collected in connection with the Contest will be used to administer the Contest in accordance with Sponsor's Privacy Policy, available at: skinrevivalscience.com/pages/privacy-policy

Submitting a nomination does not automatically subscribe the nominator or Nominee to marketing communications. Marketing emails will be sent only to individuals who separately choose to opt in.

If an Honoree voluntarily posts about the Care Package on social media, Sponsor asks that she clearly disclose that the product was received as a gift from Skin Revival Science or Cryoluxe™.

10. Use of Submitted Nominations

By submitting a nomination, the nominator grants Sponsor a non-exclusive, royalty-free license to use the nomination solely for purposes reasonably related to administering and operating the Contest. Any public use of a nomination, photograph, story, or information identifying the Honoree remains subject to the permission requirements described in Section 9.

11. General Conditions

Sponsor reserves the right to suspend, cancel, modify, or terminate the Contest if fraud, technical failure, unauthorized intervention, or another circumstance beyond Sponsor's reasonable control affects the fairness, integrity, security, or proper administration of the Contest. If the Contest is suspended or terminated, Sponsor may select Honorees from eligible nominations received before the suspension or termination.

Sponsor is not responsible for nominations that are lost, incomplete, delayed, misdirected, corrupted, or not received because of technical failures or other circumstances beyond Sponsor's reasonable control.

By participating in the Contest, nominators and Honorees agree, to the extent permitted by law, to release and hold harmless Sponsor, its parent company, affiliates, agencies, representatives, and fulfillment partners from claims arising from participation in the Contest or acceptance or use of a prize. Nothing in these Official Rules limits any rights that cannot legally be waived.

These Official Rules are governed by the laws of the State of Wyoming, without regard to conflict-of-law principles, subject to any rights that apply under the laws of an entrant's state of residence.

12. Instagram and Meta Disclaimer

This Contest is not sponsored, endorsed, administered by, or associated with Instagram, Facebook, Meta Platforms, Inc., or any other social media platform. By participating, entrants acknowledge that information submitted in connection with the Contest is being provided to Skin Revival Science LLC and not to Instagram or Meta. To the extent permitted by law, participants release Instagram and Meta from liability arising from the Contest.

13. Honoree List

To request a list of Honorees, consisting of first name and state where permitted, email support@skinrevivalscience.com with the subject line “Mother's Day, Every Day — Honoree List”. Requests must be received by December 31, 2027.
